Details of fees will be provided at the time of making an appointment. They are to be paid on the day and a medicare rebate will apply
DVA gold card holders will be bulk billed
Referrals
All patients require an in date referral whether from their GP, specialist or dentist, and to bring this to the appointment. Providers can download the referral form here.
Please bring a list of current medications, relevant investigations and x-rays to the appointments.
Sleep Patients
Patients referred for a sleep study will be seen by A/Prof Smith for a consultation before a sleep study is booked.
Children
A/Prof Smith does not see children, but may see some patients over 16
